As parent/guardian of the above named camper, I state that he/she is physically able to participate in ALL camp functions and activities. Any allergies or physical/mental disabilities will be made aware of to an Alley-Oop counselor before attending camp. I authorize any emergency treatment deemed necessary for any camper to be administrated by the staff. I also agree to hold harmless the medical staff, Alley-oop counselors, skimboard manufacturers, and/or the Delaware State Seashore Park. I have read and understand the Alley-Oop FAQ’s and policies.
